Contract 1 — Full-Time Employment Agreement (General Dental Practice) For Practice Managers, Treatment Coordinators, FOCs, and administration or clinical team members working 38 hours per week. Covers commencement and probation, position and duties, hours of work, remuneration, superannuation (including Payday Super from 1 July 2026), all NES leave entitlements (including 10 days paid family and domestic violence leave and Baby Priya's Law parental leave), public holidays, confidentiality, intellectual property, outside employment, social media conduct, AHPRA registration obligations, notice of termination table, and employee acknowledgement checklist.
Contract 2 — Part-Time Employment Agreement (General Dental Practice) For FOCs, administration, or clinical team members working regular set hours under 38 per week. Includes a day-by-day agreed hours table, pro-rata leave entitlements, and all confidentiality and conduct provisions by reference to Contract 1.
Contract 3 — Casual Employment Agreement (General Dental Practice) For administration or clinical team members with no guaranteed hours or regular pattern. Covers the nature of casual engagement, 25% casual loading, casual conversion obligations under the Fair Work Act 2009 (including the requirement to provide the Casual Employment Information Statement), and termination provisions.
Contract 4 — Full-Time Dental Assistant Employment Agreement Specifically designed for Dental Assistants and Oral Health Therapists in a clinical support role. Includes a critical alert on the new DA classification structure under Schedule J of the HPSS Award effective 1 April 2026 — with guidance on confirming the correct classification and applying the updated minimum pay rates. Covers DA-specific duties (chairside assistance, sterilisation, radiographic assistance), PPE and uniforms, and CPD support.
Contract 5 — Part-Time Dental Assistant Employment Agreement For DAs working regular set hours under 38 per week. Includes the same Schedule J classification alert and agreed hours table, with all other terms by reference to Contracts 1 and 4.
Contract 6 — Associate Dentist Independent Contractor Service Agreement For genuine independent contractor arrangements with associate dentists. Includes a clear legal warning on the risk of misclassifying an employee as a contractor, the ATO and Fair Work penalties that apply, and the factors that determine whether a dentist is truly a contractor or an employee. Covers services, percentage-of-billings remuneration, equipment and facilities, patient record ownership, confidentiality and non-solicitation, and termination provisions.
