A Complete DISC Self-Assessment and Leadership Guide for Dental Practice Managers, 2ICs, and Their Teams

The same management approach that works perfectly with one team member falls completely flat with another. The same feedback style that motivates one person shuts another down. The same communication that feels direct and efficient to one team member feels cold and dismissive to another. This is not a personality clash — it is a style difference. And once you understand it, you can work with it.
The DISC Personality Profiling Workbook 2026 is a complete DISC self-assessment and leadership guide for dental practice teams — including a 20-question self-assessment questionnaire, a scoring sheet, full style explanations written specifically for PM and 2IC roles, and a practical team application toolkit. No external assessment tool required, no subscription, no login. Everything needed to understand your own style and your team's styles is in this one document.
The workbook is structured across 5 parts:
Part 1 — What Is DISC? A clear, practical introduction to the DISC framework — four primary behavioural styles: Dominance (D), Influence (I), Steadiness (S), and Conscientiousness (C). DISC does not measure intelligence, clinical skill, or values — it describes how people prefer to communicate, make decisions, process information, and respond under pressure. Covers the most important principle: DISC is a spectrum, not a label. Every person is a blend of all four styles. Most people have one or two dominant styles that feel natural. Understanding your own blend and your team's blend changes how you lead.
Part 2 — Self-Assessment Questionnaire A 20-question self-assessment questionnaire covering how the team member takes charge of tasks, contributes in team discussions, solves problems, responds under pressure, communicates, is motivated, prefers to work, responds to change, leads, and plans. Each question offers four options — one for each DISC style — and the instruction is to answer instinctively based on how you actually behave at work, not how you think you should behave. Designed to be completed in under 10 minutes and used immediately.
Part 3 — Scoring Sheet A simple scoring sheet — count how many times each column was selected, record the total for each style, and identify the primary style (highest score) and secondary style (second highest). No calculation required beyond counting. The result is a clear, immediate picture of the team member's dominant behavioural style.
Part 4 — Understanding Your Style Detailed style explanations for all four DISC profiles, written specifically for how each style functions in a PM or 2IC role in a dental practice. The D Style (Dominance) — takes charge quickly, thrives in fast-paced environments, leads with confidence, not afraid to address performance issues directly. Strengths in emergencies, conflict resolution, and operational decision-making. Development areas: may appear blunt or impatient, might overlook team emotions. The I Style (Influence) — builds rapport easily with patients and staff, motivates with enthusiasm and warmth, naturally boosts morale and team culture. Strengths in reception leadership and team building. Development areas: avoids conflict, may overpromise and struggle with follow-through. The S Style (Steadiness) — provides stability and calm during busy periods, deeply supportive and dependable, acts as the glue that holds the team together. Strengths in patient care, treatment coordination, and continuity. Development areas: avoids difficult conversations, hesitant in fast-paced decisions. The C Style (Conscientiousness) — ensures accuracy, compliance, and high-quality care, sets and maintains systems and processes, brings order and structure. Strengths in infection control, compliance, reporting, and data accuracy. Development areas: can be overly critical or perfectionistic, struggles with ambiguity. Each style includes a practical "how to work with this style" guide for the PM — covering communication approach, feedback delivery, motivation, and what to avoid.
Part 5 — Team DISC Map and Application A team DISC map template for recording every team member's primary and secondary style, key motivator, communication preference, and PM notes — a leadership awareness tool, not a labelling exercise. Includes a practical application table covering five common PM situations across all four styles: giving feedback, introducing change, resolving conflict, motivating under pressure, and performance conversations. Each cell gives a specific, actionable approach for that style in that situation. Closes with the most important insight in the workbook: DISC-style clashes are one of the most common causes of team conflict. A high D and a high S will clash on pace and directness. A high I and a high C will clash on detail versus energy. Understanding the style difference does not excuse the behaviour — but it helps the PM facilitate resolution more effectively.
Who this is for: Practice Managers who want to understand why their team members respond so differently to the same management approach — and want a practical tool to adapt their leadership accordingly. 2ICs developing their leadership skills and wanting a framework for understanding the people they work alongside. Practice Owners who want their PM to have access to a practical, immediately applicable people leadership tool without the cost of an external DISC assessment provider. Any dental practice team that wants to do a team workshop on communication styles and work better together.
📄 Format: 7-page editable Microsoft Word document (.docx) — print one copy per team member for the self-assessment, complete the team DISC map together, and use the application table as an ongoing leadership reference. Designed to be used as a team workshop tool or as a self-directed leadership development resource.
